Person centred planning ensures you are central and in control. Plans are flexible, setting no limits to your wants, needs and dreams for your life.
A plan should be directed by you, and underpinned by your right to have control over your own life.
It should also consider and respect your family, and other people who are important in your life.
Some everyday examples of the way we operate in a person-centred way include:
- Helping people to "dream" of the future they want
- Helping them form a plan to realise their dreams
- Supporting people to make key life transitions, such as moving to fully independent living or taking up employment
- Supporting people to volunteer or otherwise be involved in their community
- Helping someone choose, plan and enjoy their holidays
